Service Coordination services are provided to link individuals to needed supports and to coordinate services. Service Coordinators assess individual needs, facilitate development of individual service plans, arrange for services and monitor the provision of those services. Service Coordinators often times assist families with questions, planning or referrals on subjects such as Social Security, guardianship, Medicaid, waiting lists, etc.
The Service Coordination Department receives reports of major unusual incidents (MUIs) involving individuals who have mental retardation or other developmental disabilities. Service Coordinators participate in investigations and monitor individuals to minimize or prevent further incidents.
